Student Vaccination Documentation

Dear Students, 

As the College communicated in early May and as part of our Fall 2021 plan to return to a robust interactive student experience, including in-person instruction and on-campus activities, it remains our intention to require that all students returning to campus in fall 2021, including those studying in Montreal and Dublin, be fully vaccinated once the vaccines have received full FDA approvals.  

The U.S. Food and Drug Administration is currently evaluating Pfizer and Moderna’s request for full FDA approval, and we anticipate that this approval will come through this summer. When one vaccination has received full approval, the College will begin requiring vaccination and will accept any vaccine currently approved for use in the United States. Given that it can take up to six weeks to achieve full vaccination depending on the vaccine received, we strongly encourage you to start this process now if you have not already done so. 

Please find detailed information below describing how to provide documentation of COVID-19 vaccination to the Student Health Center as well as how to seek qualifying exemptions for medical or religious reasons. If you have already submitted your COVID-19 vaccination to Student Health and we have confirmed receipt, you do not need to submit your vaccination record again. 

The COVID-19 vaccine has been shown to be safe and effective at preventing the transmission of COVID as well as preventing serious illness or death from COVID-19. For complete details on the benefits of the COVID-19 vaccine and how to schedule an appointment, please refer to the CDC website.  

Instructions for RETURNING students to complete your COVID-19 immunization requirement documentation:

  • Go to champlainportal.pointnclick.com and click “Sign into Champlain using Champlain”
  • Enter your Champlain MyMail username and password
  • Enter your date of birth and click “Proceed”
  • You will see several menu options on the left side of the screen. Begin by clicking “Profile” and confirming that all of the information is accurate
  • It is optional to select “Medical Clearances” and “Health History.” This section includes several consent forms and your health history; however, this information is optional as we did collect this information from you when you first started at Champlain, prior to our implementation of this Electronic Health Record. 
  • Next, select “Immunization Records.” You are required to upload a record of your  COVID-19 vaccination record here. You are not required to enter information for the immunizations you provided to us when you entered the College, only your COVID-19 vaccination record. 
  • After uploading a PDF or photo of your vaccination record, please enter the dates you received your COVID-19 vaccination(s) by clicking the specific vaccine name. Student Health will review your information and contact you if more information is needed for clearance.

Instructions for INCOMING students to complete your vaccine requirements, including COVID-19 immunization requirement documentation:

  • Go to champlainportal.pointnclick.com and click “Sign into Champlain using Champlain”
  • Enter your Champlain MyMail username and password
  • Enter your date of birth and click “Proceed”
  • You will see several menu options on the left side of the screen. Begin by clicking “Profile” and confirming that all of the information is accurate
  • Next, select “Medical Clearances.” This section includes several consent forms, your health history, and immunizations
  • After you complete the consent forms and health history, select “Immunization Records.” You are required to upload a record of your immunization history, as well as your COVID-19 vaccination documentation. If you do not have your complete immunization records, you may download and print the form and have it completed by your provider, then upload it. The required list of required vaccines in addition to COVID-19 is included on the form and our website at champlain.edu/health-FAQ
  • After uploading a complete vaccination record, please enter the dates you received each required vaccination by clicking the vaccine name. Student Health will review your information and contact you if more information is needed for clearance

To seek exemption for the COVID-19 immunization requirement for qualifying medical or religious reasons:

Student Health will accept COVID-19 vaccination documentation on a rolling basis through the summer; however, we strongly encourage you to submit your documentation as soon as possible. More information will be forthcoming over the next few months as we plan for an interactive in-person campus experience this fall.
